# PhUCKIT RC30 — ambulatory / acute medication reconciliation

## Capture and Med Rec maintenance update

- Medication-grid rows with missing doses or no local-library match remain visible as explicit review-gated records instead of disappearing from the capture.
- Standardized medication records retain raw order text plus dose, concentration, route, formulation, SIG/frequency, PRN, status, start/stop, last fill, days supply, refills and order type when visible.
- Ctrl+Space now opens the active patient Med Rec workspace. The comparison presents ambulatory and acute orders side by side and flags dose, route, frequency/SIG, formulation, status, start/stop, additions and omissions for pharmacist review.

Released September 25, 2026. Build 2026.09.25.30.

- Exact-regimen matching runs across all medications before unique same-ingredient changed-regimen matching. Multiple candidates remain visible instead of being paired by list order.
- Same-class substitution candidates do not consume acute orders or imply equivalence.
- Missing dose, route, frequency and status are explicit evidence gaps. Held, stopped, pending, PRN and formulation/concentration differences remain visible.
- Same-ingredient repeats prompt review of split regimens, duplicate captures and duplicate therapy.
- Both profile-completeness confirmations affect absence wording. A partial ambulatory capture no longer implies that an inpatient order is new.
- Search includes findings; filters cover discrepancies, each one-sided profile, matched and review state. Selection survives refresh; review decisions are timestamped.
- Complete comparison export includes all rows, completeness and decisions, independent of display filters.
- Background admission comparison uses the same engine without inventing active status. Capture signatures retain full-text/formulation distinctions.
- Existing supervised mCDS Continue, fill-date submission and eye-drop automation are preserved.

## Verification and limits

28 focused reconciliation assertions plus the existing regression suite. Includes input reordering, split regimens, ambiguous candidates, missing fields, modified release, PRN, status changes and both completeness directions. No live patient profiles were used.

This release has not been validated against your live facility EHR. Identical repeated source lines cannot establish whether separate orders exist without source identifiers. Full SIG instructions, actual home use, clinical intent and therapeutic substitution still require source review. The tool does not claim comprehensive interaction screening or automatic therapeutic equivalence.
